Athletic Registration

Step 1: Register Online

(Required) All students must complete the Athletics Online Registration.


Step 2: Turn in Paperwork

(Required) All students need to have current paperwork on file prior to participation. Required paperwork can be accessed through the links below. Completed forms can be scanned and emailed to cheryl.johnson@district6.org or turned in by the student in their assigned classroom, during their assigned “on campus” week.

  1. Sports Physicals – Required every 2 years
  2. AAPP Emergency / Insurance Required EVERY year
  3. Code of Conduct – Required EVERY year
