Professional Development Request Form

CPSD6 offers many professional development opportuntities for our teachers, including District Team Meetings or Mentor Funds. Some are sponsored by our District and many are offered by the Southern Oregon Education District, Southern Oregon University and more.

The district agrees to provide $10,000 per year in a professional growth fund for the professional development of District 6 licensed personnel. The Labor-Management Committee establishes criteria and procedures for how teachers can access the fund. 

Please complete the form for the following request categories:

  • Teacher Choice PD: Teacher has a PD opportunity they would like to attend. Approval covers the registration fee ONLY and does not cover travel, meals, mileage or substitute costs. Teacher Choice PD is part of the Collective Bargaining Agreement for Licensed Staff
  • 2nd Year Mentor Funds: A 2nd year, new to the profession teacher, and in the district 2nd Year Mentor Program choosing PD instead of a teacher mentor. Teacher may request up to $750 for PD and our materials to support the PD. This category can be used for registration, travel costs, meals, mileage, sub costs, curriculum rate or materials.  Funds must be spent by June 1st of the current academic school year.
  • District/School PD: Teacher has been asked by their building admin or district to attend a PD opportunity aligned with our district/school goals This category covers registration, travel costs, meals, mileage, sub costs and curriculum rate if outside of your normal work hours.
    • If the teacher is attending a workshop/conference in person as required or requested by their building administrator/district, the teacher is also required to complete a per diem request form for a reimbursement check to be completed. The teacher shall turn the per diem form into their building secretary at least two weeks prior to the conference/workshop. You can also PRINT this per diem request form for the school secretary.
  • District Teams: Teacher is a member of a district team (curriculum adoption, TAG, MTSS, 504, etc…). This category covers any registration, travel costs, meals, mileage, sub costs and curriculum rate if outside of your normal work hours.

Criteria and Procedures:
   
      ------> Complete the District provide PD Request Google Form
          ------> One form per person and per PD or team event
          ------> Building administrator(s) and secretary have access to the approval log to verify approved funds
          ------> Funds will be approved for all Teacher Choice PD in two cycles:

  • July 1-Dec 31 up to $5000 first come, first serve
  • Jan 1-June 30 up to $5000 first come, first serve
